Doctors investing in real estate is not a new trend, but the way they do it has changed. A generation ago the physician landlord bought a duplex near the hospital and managed it between shifts. Today most physician capital reaches real estate through syndications, where a sponsor buys and operates the property and the doctor invests as a passive partner.
This guide explains how that structure works, what it costs to get in, what the returns have looked like, and where the risk actually sits. It is written for a physician with a high income, limited time, and a compliance officer's tolerance for vague promises.
Why physicians gravitate to real estate
The appeal is structural. A physician's income is high, taxed heavily, and almost entirely dependent on hours worked. Real estate offers the two things that income lacks: depreciation that shelters cash flow, and equity that grows without another shift. Physician Side Gigs and The Real Estate Physician both report that syndications have overtaken direct ownership among their readers, largely because nobody wants a tenant calling during a procedure.
How a syndication works
A syndication is a private offering, usually under Regulation D, in which a sponsor pools capital from investors to buy a single property or a small portfolio. Most syndications set a minimum investment of $50,000, with the sponsor as general partner and physicians as limited partners. The sponsor finds the deal, arranges the debt, runs the business plan and takes a share of the profit for doing so.
Because these offerings rely on the 506(c) exemption, most require accredited investor status: an income of $200,000, or $300,000 jointly, in each of the last two years, or a net worth above $1 million excluding your home. We do accept non-accredited investors on some deals, and the offering documents say which.
Returns, risk and the lockup
Projected returns are only ever projections. What matters is the assumptions underneath them: the rent growth, the exit cap rate, and the debt terms. Ask for all three before you ask about the internal rate of return.
Liquidity. Expect a five to seven year hold with no early exit.
Fees. Acquisition, asset management and disposition fees are normal; ask how each is calculated.
Verification. The SEC's EDGAR database shows whether a sponsor has actually filed the Form D they claim.

How the score is made, what the floors do, and what a fix is never allowed to do.
Does a model decide the score?
No. Every number is computed from the 46-check suite: the same article always produces the same score, and every point traces to a check you can open and read. A model is allowed to propose a fix or draft replacement copy. It is never the source of a score.
What are the weights and the floors?
On a YMYL site, Overall is 45% SEO, 20% AI Search and 35% E-E-A-T. If a dial has nothing to score, its weight is spread across the others rather than dragging the number down. The floors are Overall 80 and every scored dial 70, and they are the same on every plan.
What happens below the floor?
Publishing stays open, but you record a reason first and it is logged against the article. Warnings above the floor never block anything: a 93 with six suggestions is a polish list, not an obstacle. The compliance gate is the exception. A hard failure blocks, with no reason to give.
Can a one-click fix make compliance worse?
No. Before you see a proposal, the suite runs on it and compares every check with the current draft. If any compliance or integrity check worsens, or publishing would newly block, the fix is refused. Accepting re-runs the same guard server-side, saves a revision you can restore, and re-validates.
Which checks have no Fix button?
Compliance and integrity checks, because rewording a disclaimer or a claim is your judgement. Word count, internal-link count and the emitted schema, canonical, Open Graph, Twitter, table-of-contents, FAQ-schema and author-bio checks render as advisory rows, since a scoped text edit is the wrong tool for them. First-hand experience is fixed only from your own knowledge base, and declines when nothing there fits.
Why does an older article show a stale score?
The checks improve over time, and an article keeps the results from the day it was last validated. Verand hashes the validated bytes, so it knows when the draft has changed underneath the card. Click Re-validate to re-run the suite, including the live source checks behind the Accuracy meter.
