Doctors investing in real estate is not a new trend, but the way they do it has changed. A generation ago the physician landlord bought a duplex near the hospital and managed it between shifts. Today most physician capital reaches real estate through syndications, where a sponsor buys and operates the property and the doctor invests as a passive partner.
This guide explains how that structure works, what it costs to get in, what the returns have looked like, and where the risk actually sits. It is written for a physician with a high income, limited time, and a compliance officer's tolerance for vague promises.
Why physicians gravitate to real estate
The appeal is structural. A physician's income is high, taxed heavily, and almost entirely dependent on hours worked. Real estate offers the two things that income lacks: depreciation that shelters cash flow, and equity that grows without another shift. Physician Side Gigs and The Real Estate Physician both report that syndications have overtaken direct ownership among their readers, largely because nobody wants a tenant calling during a procedure.
How a syndication works
A syndication is a private offering, usually under Regulation D, in which a sponsor pools capital from investors to buy a single property or a small portfolio. Most syndications set a minimum investment of $50,000, with the sponsor as general partner and physicians as limited partners. The sponsor finds the deal, arranges the debt, runs the business plan and takes a share of the profit for doing so.
Because these offerings rely on the 506(c) exemption, most require accredited investor status: an income of $200,000, or $300,000 jointly, in each of the last two years, or a net worth above $1 million excluding your home. We do accept non-accredited investors on some deals, and the offering documents say which.
Returns, risk and the lockup
Projected returns are only ever projections. What matters is the assumptions underneath them: the rent growth, the exit cap rate, and the debt terms. Ask for all three before you ask about the internal rate of return.
Liquidity. Expect a five to seven year hold with no early exit.
Fees. Acquisition, asset management and disposition fees are normal; ask how each is calculated.
Verification. The SEC's EDGAR database shows whether a sponsor has actually filed the Form D they claim.

Which checks block publishing?
Three kinds. A failed compliance check (a required disclaimer missing or altered, a hard-tier banned claim, or a pack rule such as Reg D or HIPAA), the prompt-injection scan, and the programmatic uniqueness gate. A block sets publication_blocked on the article, and both the WordPress and GitHub publish paths refuse it. Everything else lowers a dial or lists under "to fix"; only a dial under 70 or an Overall under 80 asks for a logged override.
Why does the editor say 48 checks when this page says 46?
The Checklist counts rows, not validators. It splits the keyword-placement check into four rows, adds a row for the experience facts used and four hero-image verdicts, and leaves out checks that did not run: the pack checks for other industries, the two that only surface when they fail, and anything that returned not applicable. On the Willowdale draft above that is 48 rows over 40 validators that ran.
What does deterministic mean here?
Each check is a pure function over the article HTML and its brief: counts, regular expressions, presence, string matching. Run it twice on the same bytes and you get the same verdict. A model proposes fixes and researches targets; it never assigns a verdict, and every proposed fix is re-run through the suite and refused if a compliance or critical check gets worse.
What happens when a check does not apply?
It returns not applicable. That result is excluded from every dial, never persisted, and never shown as a pass. Term coverage, evidence inclusion, secondary keywords and PAA coverage stay inert until a researched brief supplies targets; FAQ schema, citation schema and the author-bio check stay inert when the article has no FAQ, sources or named author; the uniqueness gate only compares sibling pages in a programmatic batch.
How do you know a validation is still current?
Each run stores a SHA-256 of the exact title, meta description and body it checked. A validation is fresh while those bytes are unchanged, stale when any of them differs, and unknown for articles validated before the hash existed. Nothing infers freshness from a timestamp, so a status change or a no-op save can never flag a draft as edited by you.
Can a check be switched off or its threshold changed?
Thresholds are configuration on the resolved ruleset, not code: the coverage bar, the uniqueness threshold, the severity of your banned-phrase list, and a per-validator enabled flag. An operator can adjust them per ruleset; there is no customer-facing toggle in the dashboard today. Compliance checks and the hard tier cannot be switched off by a customer, and the gate fires identically on every plan.
